Community Pathway Connector

Community Pathway Connector is a Gold Coast Primary Health Network initiative aimed at connecting people to community and appropriate support, services and activities.

The role also includes advocating for people’s needs and navigating through systems. This service is open to individuals of all ages and no clinical diagnosis is necessary. You can contact us for an initial intake assessment and we can assist you to identify barriers and connect you with suitable support providers. MCCGC & CÜRA have a well-established network and extensive experience working with culturally and linguistically diverse individuals, ensuring that we can link you in with the services that best suit your individual requirements. We're here to help you get the support you need.

Meet Maria, our previous MCCGC Community Pathway Connector (CPC). Our team supports individual community members from multicultural backgrounds to access supports and services to improve mental and emotional wellbeing, to make connections in the community, and to feel more confident. CPC is a very flexible program that offers individualised support to meet the unique needs of each client.
